Recuperative Care Services

Recuperative Care is a short term residential care for homeless individuals who no longer require hospitalization. These individuals still need to heal from an injury or illness (including behavioral health conditions), and their condition would be exacerbated by an unstable living environment.

We provide comprehensive follow-up healthcare in a home-like facility, case management and transitional housing options for hospital-referred homeless clients in Los Angeles County.

Post Acute Medical Care

Medicaton Assistance / Management

Maintaining a consistent medication schedule is crucial to the recovery process. Our staff is well prepared to help the patients take their medication exactly as it is prescribed.

We provide comprehensive post-acute transitional care for our patients who may be recovering from illness, surgery, or other medical procedures. Homeless patients are most vulnerable when in need of post-acute care. This is the key elements to our recuperative care program.

Daily Monitoring

Our patients recieve support-monitoring from our dedicated staff 24 hours a day, 7-days a week. Our trained staff is committed to helping our patients recover quickly and find the support necessary to prevent homelessness and reintegrate into the community.

Transportation

Transportation can be difficult to obtain for those facing homelessness. Get Well Soon provides prompt and reliable scheduled transportation to and from medical, social security, & DPSS appointments for our patients.

Comprehensive Homeless Case Management

Our patients at Get Well Soon receive comprehensive home case management as part of our standard treatment process. Our staff is well-prepared to provide referral between our patients and homeless case management professionals.

Comprehensive Homeless Case Management

Mental Health conditions and Addiction are some of the major barriers faced when in recovery or breaking the cycle of homelessness. Get Well Soon partners with other community support services to provide our patients with access to behavioral health clinics, AA meetings, methadone clinics and other services as needed to prevent the cycle of homelessness.

Mental Health Assessment & Referrals

Our team of professional staff at Get Well Soon includes specialized nurses (and social case managers to help patients coordinate mental health care as a part of their recuperative treatment as needed. Mental health is an important element in the path to breaking the cycle of homelessness. Our staff is dedicated to providing our patients with the mental support they need for continued success.
